Our mission is to empower and engage our diverse communities by focusing on social determinants of health, such as housing, education, and employment, which account for 80% of health outcomes. We are committed to eliminating health inequities related to socioeconomic status, race, and ethnicity through measurable prevention, early intervention, and treatment strategies.

CCHI supports a wide range of programs that reduce social and economic barriers for vulnerable populations, including community health workers, navigators, and outreach initiatives. Our Mass General Youth Programs inspire over 1,000 underrepresented students, offering opportunities in health and science to spark college aspirations and beyond.

We lead and support four multisector coalitions across Revere, Charlestown, Chelsea, and East Boston, addressing urgent health challenges such as substance use, obesity, and trauma. Guided by a commitment to diversity, equity, and inclusion, we believe in the power of prevention and collaboration to make entire communities healthier.
