"I am a patient @ Dr Kosmary' Silver Creek Pain in Bullhead City, AZ. I have been waiting 7 weeks for the approval from insurance for an injection. They said tbey will call when approval is received. Insurance approval is requests are sent by the Las Vegas office. I have called 4 times, now, and left messages regarding the authorization of my injection. Insurance has called also and just got nowhere. Absolutely no response and Insurance has not received a request for approval."
