Important Announcement: Clinic Closure

After more than five decades of providing exceptional healthcare services, DCMG will be closing its doors on August 31, 2024. Our last day of scheduled patient care will be August 28, 2024. This decision aligns with our parent company, GuideWell's, strategic focus on delivering high-quality and affordable primary care through Florida Blue | Sanitas Medical Centers.

Next Steps for Patients

- Finding a New Doctor: Please contact your insurance company for assistance in finding a primary care doctor or specialist within your network. Use the phone number on your insurance card or visit your health plan's website for provider directories.

- Medical Records: Once you have chosen a new healthcare provider, you can request your medical records following the instructions provided on our website.

- Prescriptions and Referrals: We will continue to manage your prescriptions and referrals until our last day of operation. Your new primary care doctor will help you with any future needs.

Introducing Florida Blue | Sanitas Medical Centers

Starting September 3, 2024, four new Florida Blue | Sanitas Medical Centers will open at or near the former locations of DCMG clinics. These centers are dedicated to providing seamless, value-based care to Florida Blue members, offering personalized primary and walk-in care, as well as on-demand virtual support.
