After decades of service, DCMG will be closing its doors on August 31, 2024, with our last day of scheduled patient care being August 28, 2024. This decision aligns with the strategic direction of our parent company, GuideWell, emphasizing seamless, high-quality, and affordable primary care for Florida Blue health plan members through Florida Blue | Sanitas Medical Centers.

Continuing Your Care:

We understand the importance of continuity in healthcare and are committed to supporting you during this transition. Please note the following steps to ensure your ongoing care:

1. Finding a New Doctor: Contact your insurance company using the phone number on your insurance card to identify in-network doctors. Your health plan's website may also provide a directory of available providers.

2. Medical Records: Once you've selected a new healthcare provider, you can request your medical records to be transferred following the provided instructions.

3. Prescriptions and Referrals: We will continue to assist with prescriptions and referrals until our closing date. Your new primary care doctor will guide you in obtaining necessary referrals or authorizations thereafter.

4. Urgent Care Needs: If you require medical attention after our closure and have not yet transitioned to a new primary care doctor, visit an urgent care facility within your plan's network. In case of an emergency, please call 9-1-1 or proceed to the nearest emergency room.

5. New Florida Blue | Sanitas Medical Centers: Starting September 3, 2024, four new Sanitas Medical Centers will open near the former DCMG locations to serve Florida Blue commercial members, offering comprehensive primary and walk-in care.
