"I visited a long-term care patient at this facility. The plus is that the patient has shelter and is under observation. The negative is the poor meal choices (see pics), very old room furniture/bed (late 80s/90s), access to the main entrance is not manned, making it nearly impossible to have a visitor unless the patient is well enough to meet you at the door or you catch another resident outside who will let you in. The patient will waste away if no one goes to visit. The patient will definitely need your care."

"0/5 I have to give 1 star simply because it wont let me put zero. I'm a patient at this facility not long term. Between being give wrong medications, medical and admistration mistakes, what should have taken 6 to 8 months to recover after major back surgery, has blown up into not being able to walk like a normal human being after 13 months being admitted to Park Center for post surgical rehabilitation. 4 out 13 months I have been at Park Center were spent in the hospital due to medicine and medical mistakes made by the caring doctors at the facility. At one time having to be transported to the ER because a medicine mix up caused me to stop breathing. And today still suffering from after effects due to that mistake. The facility has a prescription for physical rehabilitation given to me by a doctor. Yet Park Center intend to discharge me to a program that has no physical rehab training. The food in the facility is below every standard and Park Center has numerous violations from ODH regarding this. Of which Park Center refuses to improve or change. I have several documented issues were Park Center representative have given misinformation, incorrect information or false/falsified information. To myself, insurance companies, the states of Ohio and Pennsylvania, were I am actually a resident of. I will advise the beautiful photos on Park Centers web page are not any of the actual facility. The facility is dirty. Things as simple as elevators that do not work correctly to doors that should work automatically that rarely do. I wouldn't recommend this facility to my worse enemy. There several facilities in the Youngstown area that are owned by the parent company that owns Park Center. With what I have learned those facilities are operated in the same lack luster fashion. I would suggest doing your home work before allowing any family member or your own self to be admitted to any of their facilities. The web page indicates they have been in business in the Youngstown area since 1964. When in fact they have only been at this location since 2020. ************************ I searching for any individuals who are considering or in legal proceedings against Park Center."

"My grandma just recently got placed there and I was worried about staff being rude towards her and everything. Turns out the staff was very friendly and helpful. I had asked one of nurses about my grandma and how she’s been. The nurse explained everything that was going on and her progress and was very nice about it. I couldn’t find my way around the building and the nurses helped me find my way around. Overall I had a good experience there! Edit: I spoke too soon with this place. There were a few occasions they’d transferred my grandma to the hospital and not inform anyone until we showed up to visit and she wasn’t there. They actually never rotated her or checked her oxygen which caused a lot of harm for what she was dealing with and this place is literally why she went into hospice and sadly passed away. My grandma said (before she passed) that the staff was nice but didn’t care for her well being, if family hadn’t been there as often as we were I don’t think she would’ve made it even a week in that place."

"My family and I went there to help with a Christmas party. It was a lot of fun. It had music, and many gifts to those that live there. The staff at this place definitely is great with the residence of this place. Their actions spoke louder than anything else. I was able to play Santa clause there as well . I was honored to participate at this time event."
