"Seemed like a nice lady at first when i was trying to become a new patient. Denied service because the only way she could accept service is if the forms were filled out in person. I attempted to email, text in multiple forms and she said I wasnt cooperating. She originally advised she could potentially accept this way but had trouble opening and denied service. Very disappointing for someone who is looking for help. She sent me a message saying i had a bad attitude when i asked her if she received the forms."
