Crystal Lake Medical Arts - Centegra Health System

"My name is Mark McFarland. I have received three preauthoziations from the office of Dr. Holly S. Carobene for procedures I am NOT having. I live in Colorado; I have never been to Illinois. I have tried to reach the office of Dr. Carobene, but no one will give me the telephone number. Other phone contacts with Crystal Lakes Medical Arts and Northwestern Medicine have been ineffective. No one can even find me in the system; yet I have three pre-authorizations in my Blue Cross in-box and have received on letter by mail. So, you have my personal identification information, my BCBS ID number and other private information. I have contacted BCBS of Texas, Crystal Lakes, Northwestern Medicine, and AIM Specialty Health (a contractor of BCBS) and have not been able to get this resolved. I now have filed a formal Fraud Complaint with Blue Cross Blue Shield. The next step will be to file a criminal complaint with law enforcement to prevent being charged for these services which I have not requested and will not have. Please contact Blue Cross Blue Shield of Texas to correct these errors. My dob is 7/18/58. I have made every effort to get this resolved. Please take action immediately."
