"Request to correct what Shane did with attorney response: 1. Provide a notarized certified copy of 178 pages of records that have been designated as the complete record set maintained by Shane Raymond and Brandon Mental Health, LLC, to me. Has been done, with the exception of the providing the notarization. 2. Maintain the 178 pages of records as the complete record of services provided to me by Shane Raymond and Brandon Mental Health, LLC. Goes along with #1. Moving forward in the future, when records are requested, if records are requested, and only if authorized for release, 178 pages will be provided. 3. Provide a notarized letter voiding the records that were created, backdated, and inappropriately altered with an attached voided copy of the records to me. To ensure that we are on the same page, forward a PDF of the records you wish to be voided. I know you’ve done this previously, or likely have done so, but so I have it in front of me in one discrete as an attachment, please send, noting the subject line of the e-mail, “VOID”). A letter will be provided not that the records attached to /enclosed with the letter are void. 4. Void the created, backdated, and inappropriately altered records and put safety measures in place at Brandon Mental Health, LLC so the voided records are not released in the future. Goes along #1 and #2. Moving forward in the future, when records are requested, if records are requested, and only if authorized for release, 178 pages will be provided. I’ve let you know the system that is in place. 5. Send and copy me on a letter with records attached that are to be voided to Christina Shideler at the Department of Health requesting the created, backdated, and inappropriately altered records be voided. This letter will be placed in file at DOH. See #3. Will be done but will NOT be done until the DOH approves the current agreement in the pending matter. I believe the hearing is being scheduled for May 2023. 6. The voided records are not to be released under any circumstance in the future. Goes along with #2. 7. All authorizations for BMH to release records are to be revoked immediately once this matter has been resolved. Agreed, and dovetails with #1 and #2 and #4. "Shane had the opportunity for over 2.5 years to privately correct the records he created (fabricated), backdated and altered and provided to numerous third parties after he quit. But he, his attorney or both chose to drag it out and cause severe distress. What he did has to be the most selfish behavior I have ever seen and not the behavior I would expect from a social worker who is supposed to help people and not cause them harm. He now voided the records but because he fabricated and altered the records they no longer match his billing records or insurance claims. I have requested to inspect the billing records so he can correct. I deserve to have EVERYTHING corrected. So far he, his attorney, or both have refused to allow me to inspect the billing records in EHR and what he claimed to insurance. He had the opportunity to amicably resolve and to do it privately but Shane doesn’t want to take any personal responsibility for what he chose to do. I will continue to stand up for myself and what is right. What he did is wrong and his continued behavior of not accepting responsibility and not just correcting what he did without me having to continue to request over and over again and make complaints to DOH and HHS over and over again is egregious. The psychological damage he has caused is beyond egregious."

"More correspondence I received wanting me to sign a release of all liability. I am in no way asking you to reward behavior. Far from it. I'm only asking so that all can walk away from THIS situation and so that you can feel protected. I can't change behavior for anyone for anything, but the hope is that experience guide actions in the future. That applies to everyone for all things. You know what the terms of the settlement agreement is with DOH and what it is designed to do to affect future patient care. He wants you to do something. You think he's obliged to do that something to correct that something that you think is wrong. He disagrees with the premise. He thinks you're obliged to do that something to correct that something that he thinks is wrong. You disagree with a premise. That equals a dispute. The parties climb down from their lofty perches, put aside their disagreements, compromise, and voila, settlement. And the parties walk. Never to be in fear for their safety."
"Horrible customer experience. You can’t make an appointment over the phone. You have to fill out an online form with all of your information to REQUEST an appointment and then several hours later you get an auto-generated email that says they may or may not get back to you. People are reaching out because they need help. This process for “caring” for your clients feels terrible."
