"Needed a return to work form and sent it to the practice a month before I was scheduled to return to work. He did not get the form back to me and I spent the last few days of my maternity leave trying to track down the form. I called ten times uploaded it and followed up repeatedly. Was told over days that Dr Termin had it to sign and despite telling them that I needed it to return, he just took his sweet time signing the form. No reason for him not to sign it Dr Termin just had no regard for the fact that I could lose my job if I didn’t have the form. The day before I was scheduled to return I still didn’t have the form. Had to go to the office and still wasnt helped. I’ll be leaving this practice."
