"If I could give them a zero I would. My mom was put in there for 21 days for rehab from a UTI she suffered from and fell just this past month in January. Out of those 21 days she was put in Covid insulation because she had not had the Covid vaccine. We were told when I signed papers for her that we would be contacted in 2 to 3 days from the care team there and go over their plan for my mom. 10 days go by and no phone call, plus I had been calling to check on my mom and left several messages and never received a phone call back. Keep in mind I had no idea if my mom was even still alive because I could not contact her. On the 11th day I called again and again no answer including me calling the Administrator and her name is Barbara Boller who my wife and I originally talked to the night she was admitted there and we filled out the paper work with. Still no answer from her even until I left a message and told them if I did not hear from someone today I would be coming there later that day to sign my mom out. 10 minutes later I finally got a phone call back. The other issues were they never changed her clothes the whole time she was there and never gave her, her medicine she had to have everyday, which I also stressed to Barbara Boller was very important that she gets. In those 21 days my mom told me that she had only been to Rehad 2 days and the rest of the time she was never gotten out of bed. When we were finally allowed to go see her there was soft drink cans lined up on her table from where she had been given nonstop. Again she was put in there for rehab from a sever UTI she suffered from. There is so much more that I could write, but let's just say that I am calling the CMS Federal Government and our attorney and I'm going g to make it my mission to have this place investigated for Fraud and get this place shut down."

"Terrible place, my grandmother currently being housed there temporarily, she is being double roomed with dementia patients, they are missing her medications, not administering them at all at times. We can’t go in, we are fortunate she is in good enough health that she can call us and tell us. Currently she has not had her breathing medicine and has called us, we cannot reach anyone in the facility at all on a Monday August 30th. They lie about giving medicine and falsify the records to show that the medicine was administered. They cannot control elderly Covid positive dementia patients so they roam freely, spreading Covid to other patients. Federal charges should be brought against them. Maybe someone will reach out but I highly doubt it."

"This was absolutely the worst experience we’ve ever had dealing with an institution in mine and my father’s life. The communication from the nursing staff to me about what was going on with my father was non-existent. (There were a few helpful people, such as the therapists and a couple of day nurses, but they were definitely in the minority and I felt sorry that they were even working in such an environment.) My father, who was never incontinent before, had staff tell him just to “go” in his pants and they would clean it up later, just because they didn’t want to deal with the time it took to help him to the bathroom. They also took all his clothes away, in order to keep confined to his bed. It was degrading and horrible, and my poor father was so relieved and cried tears of joy when I was finally able to have him released from this place. Please don’t let your loved ones fall victim to them."

"My father in law is in this Godforsaken facility now. They will not respond to email or calls! His phone has been stolen! Next step calling the police as their home office in Birmingham is clueless. During COVID we expect more communication as no one is allowed in this facility!"

"They were very patient and efficient, taking very good care of my Mom as she stayed there for physical therapy after surgery. The team meeting with the staff at the end of her stay, which went over her care needs going forward, was very impressive. It was evident that they had evaluated Moms needs and my concerns thoroughly. We will be choosing FHR going forward as our extended caregivers and therapists as the need arises."
