We regret to inform you that the Diagnostic Clinic Medical Group, including the practice of Dr. Flores, will be closing its doors on August 30, 2024. Your health and well-being remain our utmost priority. We understand the concerns you may have regarding the transition to a new primary care provider. Rest assured, our team is here to support you in this process. Until our last day of operation, we will continue coordinating your medical care, including tests, prescriptions, and referrals.

Transitioning to a New Provider

To facilitate a smooth transition, we encourage you to work with your insurance company to find a new doctor within your network. You can contact the number on your insurance card for assistance. Once you have selected a new provider, you can request the transfer of your medical records by following the provided instructions.

Future Care Options

Starting September 3, 2024, four new Florida Blue | Sanitas Medical Centers will open near the former DCMG locations, offering high-quality primary care services to Florida Blue members. Sanitas is renowned for its value-based care model, ensuring patients receive comprehensive and cost-effective healthcare.
