Age-Friendly Care at HarmonyCares focuses on the 4M’s: understanding what Matters most to you and your family, safely managing Medications, enhancing Mobility around your home to reduce falls and injuries, and focusing on the Mind by monitoring cognitive function and identifying signs of depression associated with aging.
Our highly trained clinical staff is equipped to assist with complex medical needs and patient support, ensuring respectful care that honors individual preferences. We engage our patients as active partners throughout the course of their care.
HarmonyCares is one of the nation's largest house call practices, with services available in 15 states. Our expert team provides tailored in-home doctor visits for individuals with complex health conditions, enabling them to remain independent at home. Our CAP-accredited lab performs over 3 million diagnostic tests annually and can conduct lab tests and X-rays in the home to gather essential health information.
We focus on relieving the stress of managing illnesses, maximizing patient and family satisfaction, and minimizing hospital visits. With a doctor's order, we offer nursing, physical therapy, and occupational therapy services in the home. Proudly accredited by CHAP, our services grant access to skilled healthcare at home.
Additionally, HarmonyCares provides patient and family-centered, CHAP-accredited hospice services to help individuals continue living at home with dignity and comfort.
