Welcome to Dr. David L. Nelson, DO, your trusted healthcare provider located at 42 Park Place, Pawtucket, Rhode Island. As a federally qualified Patient Centered Medical Home, we proudly serve the communities of Pawtucket and Central Falls with a comprehensive range of affordable healthcare services. Our culturally diverse health center offers Pediatrics, Internal Medicine, Family Medicine, Obstetrical and Gynecological Services, as well as Dental, Optometry, and Behavioral Health Services.

Our mission is to deliver the highest quality healthcare at the lowest possible cost to our community members. We are currently accepting new primary care patients for all services, and our Express Health clinic in Central Falls is available for walk-in, same-day sick appointments for new patients.

In response to the ongoing Covid-19 pandemic, we conduct screenings for symptoms and request that all patients experiencing upper respiratory symptoms, such as coughing, sneezing, congestion, runny nose, fever, or sore throat, wear a mask during their visit.

We are dedicated to ensuring all income-eligible children under 19 in Rhode Island have access to healthcare through RIte Care (Medicaid) Health insurance coverage, regardless of immigration status. This coverage includes doctor’s visits, wellness exams, prescriptions, mental health care, hospital care, and more, without impacting immigration status or applications.
