"My son Dylan was a Resident in Richmond Hill Queens in one of their homes that TSi rented out from a private homeowner. Long story short TSI in 2020 had rented out a home where a person on parole next door was offering drugs for free to the residents in TSi care. The dealer would get them hooked knowing that these people in the home are on social security. Long story short my son was poisoned by fentanyl. He died in that house no emergency procedure no narcan. I got the dealer who killed my son. I never even got I'm sorry for your loss from TSI. Don't send your loved one here. They need to shape up their procedures and really have some emergency training for the people who are getting paid for renting out their homes . Fentanyl is #1 cause of death of young adults more than covid!. I wish I knew better I trusted a relative that got my son in here as a favor. My son was safer in the streets. Shame on you!You had no emergency procedures. My son was brain dead due to being on the floor for quite some time not to be found.I really need to contact an attorney. My son wasn't an addict he was left to die in his room. One pill or one line Fentanyl kills. 300 people a day are dying it's in pills weed cocaine everything watch your family. This facility should of had narcan."
"Do not send your loved ones to this garbage. Especially phase I. For starters your loved ones will have to beg for food if they get hungry after breakfast lunch or dinner because no staff wants to bother with getting up and giving them a sandwich or fix them up a plate. Secondly they treat these individuals as if they are animals. Then they tell you the client has the right to refuse medication but penalize the counselors when the client wont take their medications for more than 2 days. The director of phase I is the worst. Her name is Barth Mars and she has no care for the clients or staff. I was personally attacked by a client I called her office for help because and she refused to help me or ask another staff to come to my aide. They suggest you put the phone on intercom but the phones dont work. Then she also makes you write false notes just so she can meet her quota. And if you dont write the note because you dont want to commit Medicaid fraud and you really couldn't meet with the client she writes you up for insobordination . When the director Mr. Mathew was there things were much better. Take a look and notice how she barely has any full time staff left. Ask the families of those individuals how many times they havent made complains to her and she does nothing. This is not a place to work or to live. Run while you still have a chance."
