"My 18 year old daughter had an accident at work and we took her to Banner urgent care and everyone there was very nice and professional. The doctor was very nice and calmed my daughter down. She gave her three stitches and prescribed an antibiotic. She said we would need to go to Banner Occupational Health to get the stitches removed in 10-14 days. We made an appointment online for 5:00 on December 26th but when we arrived they were closed. The next day we went in and they said they were going to lunch. They told us to sign in and sit down. We waited 30 minutes for them to come back from lunch. The guy behind the counter had my daughter fill out some paperwork and asked me to go next door and ask for copies of her records. I went next door and the girl said, why is he sending you? She said she would bring them over. When I went back the guy behind the counter said he needed a couple signatures from my daughter, he then said “ okay go ahead and sign on the pad” when I asked what she was signing he seemed to get very bothered and said something like it’s a form 109,. I asked what it was and he just said it’s a form they needed. He wanted a second signature and I asked what it was for and he sarcastically said it’s a release form. When they called her back I walked back and they said I couldn’t go back, when I questioned why she said it is up to the individual provider and said she would ask. She came back and said he didn’t want me to come back. This was one of the most unprofessional experiences I have ever had in any medical office"
