"Am a hospital pharmacist who intends to discharge counsel the patient, and called the home pharmacy to put a patient on the radar for extra counseling. Spoke with the tech who was great. Was called back by the pharmacist who spoke aggressively toward me as if I was placing blame for the patient's admission- not my intent at all. I assured him I didn't mean to offend him, I was just trying to relay that the patient may need more reinforcement. He reiterated to me several times that he spends all kinds of time with this patient, etc. I told him I didn't know this and never meant to insinuate that he was to blame, only that the patient may need education at discharge (me) and when medication dispensed (him). Again stated I didn't mean to offend him and was just trying to pass along information. He seemed very offended and frustrated.. I was very flustered and sad after this encounter. We're supposed to be on the "same team"."
