"“I am being treated at Mercy Health Physician Partners physician office/clinis (“Physician Office”), and I consent to ALL MEDICAL AND SURGICAL CARE, EXAMINATIONS AND TESTS DETERMINED BY THE PHYSICIAN OFFICE TO BE NECESSARY FOR ME.” How on earth do you get anyone to sign this? I brought my infant to Mercy Health to see Dr. Kara Morley-Smolek for his 9 month well-visit 3 months ago. While there, my husband and I read this and crossed out the above section before we signed because we agreed with everything else but not this. They accepted our form and saw us. Today for my son’s 12 month well-visit, I had an appointment. I drove 20 minutes to their facility (6050 Northland Drive) and had a terrible experience. First of all, there are no signs indicating where to park for the patient entrance so I parked on the wrong side of the building and had to walk all the way around with my infant son, hands full, in the cold. Secondly, when I arrived for my appointment, I was greeted with another registration form. The woman told me that I needed to fill out the registration form because last time we crossed out part of it. I told her that I didn’t agree with that part of the form and she told me, “then we can’t see you.” That would have been nice to know BEFORE I drove all the way over with the expectation that they had accepted our form last time and that we were good to go. Otherwise, why would they have let me make another appointment, and called me to confirm the appointment if we weren’t cleared to go ahead with the appointment? Why wouldn’t SOMEONE have bothered to mention to me something about the registration form? If you do go here, I would think twice about signing it the way that it’s worded because you are literally giving away your consent for “ALL MEDICAL AND SURGICAL CARE, EXAMINATIONS AND TESTS DETERMINED BY THE PHYSICIAN OFFICE TO BE NECESSARY TO ME.” That’s an awful lot of trust to be placing in a complete stranger and/or industry. No way am I going to give that much consent all at once for anything regarding my child."
