"I am a care provider for my sister-in-law who has Down's syndrome. I got a recommendation to this clinic and tried to schedule a new patient appointment. They wouldn't do it without me first filling out their new patient paperwork. They did not have the forms online, and did not have the ability to email me the forms. After receiving the forms in the mail, I attempted to stop by the office to drop off the paperwork and schedule an appointment. I was met with more forms to sign (which would have needed to be signed by my husband as he is the official guardian). They would not schedule the appointment without this Medical Release paperwork being signed. Again, I'm not her legal guardian, but I do have access to her electronic medical record from her old practice and could email all the records they'd need. The receptionist didn't seem to understand how electronic health records work and just kept telling me she couldn't schedule an appointment without the release form. Taking care of another person is hard enough without care providers putting unnecessary impediments in the way to taking care of someone. There is no reason why this paperwork was necessary to simply schedule the appointment. I will never know if this is actually a good doctor or not. What I do know is I will find another office that is actually interested in having a new patient."
