Located at 1301 2nd Avenue Southwest, Largo, Florida 33770, Stenzel Michael J MD has been a proud member of the Diagnostic Clinic Medical Group (DCMG), providing exceptional healthcare services to our community for over 50 years. However, we regret to inform you that after decades of dedicated service, DCMG will be closing its doors. Our last day of operation will be August 30, 2024, with the official closure date on August 31. We understand that this may raise questions and concerns, especially regarding the continuity of your healthcare.

Continuing Your Care:

Your health and well-being remain our top priority. We are committed to ensuring a smooth transition for all our patients. Here’s how we plan to support you:

- Finding a New Doctor: Please contact your insurance company using the phone number on your insurance card to find a new primary care doctor or specialist within your network. Your insurance provider will assist you in locating suitable healthcare professionals in the area.

- Medical Records: Once you have selected a new doctor, you can request your medical records following the provided instructions. We will ensure that your new healthcare provider receives all necessary documentation to continue your care seamlessly.

- Prescriptions and Referrals: We will continue to manage your prescriptions and referrals until our closing date. If you require a new or refill prescription, please contact us before we close. Your new primary care doctor will assist with any future prescriptions or referrals.

- Urgent Care Needs: If you have not yet found a new primary care doctor after our closure, you may visit an urgent care facility within your plan’s network, such as GuideWell Emergency Doctors (GEMD). For medical emergencies, please dial 9-1-1 or visit the nearest emergency room.

Transition to New Care:

After our closure, four new Florida Blue | Sanitas Medical Centers will open near the current DCMG locations starting September 3, 2024. These centers will provide comprehensive primary care services to Florida Blue members.
