After decades of service, we have made the difficult decision to close our clinics. Our last day of operation will be August 30, 2024. This decision aligns with the strategic direction of our parent company, GuideWell, focusing on delivering seamless, high-quality, and affordable primary care through Florida Blue | Sanitas Medical Centers.

Continuing Your Care:

Your health and well-being remain our top priority. We understand the transition to a new primary care doctor or specialist may be concerning. Rest assured, we are here to support you during this change. Our team will continue to coordinate your care, including tests, prescriptions, and referrals, until our closure.

Next Steps for Patients:

- Finding a New Doctor: Contact your insurance company using the number on your insurance card to find doctors within your network. They can assist you in identifying a new primary care provider.

- Medical Records: You can request your medical records following our provided instructions.

- Referrals and Prescriptions: We will coordinate any necessary referrals and prescriptions until our last day.

Future Care Options:

Starting September 3, new Florida Blue | Sanitas Medical Centers will open near our former locations to continue serving Florida Blue members. Sanitas is renowned for its value-based care, offering personalized and comprehensive primary and walk-in care, along with on-demand virtual support.
