Lewis in 2004 with the vision of providing mental health care services to all individuals in need, including the underserved and uninsured. Our dedicated staff strives to deliver the level of care that Doreen Lewis intended, serving the mental and physical health care needs of individuals in all communities.

Our providers work with patients to develop personalized treatment plans to achieve optimal mental health goals, offering services such as video conferencing for those with transportation difficulties, pharmacy coordination, medication management, and primary care coordination. Additionally, our medical providers focus on prevention and offer services such as immunizations, lab work, chronic disease management, and screenings for all ages.

Specializing in addiction and behavioral health management, our staff also provides services such as group counseling, marriage and family counseling, substance abuse treatment, and case management. We have dedicated teams throughout Southern Arizona working with individuals involved with adult probation.

Our mission at Community Health Associates is to provide Arizona communities with high-quality behavioral health treatment services, substance abuse rehabilitation, and prevention services that respect the culture and ethnicity of our members. "My grandson, who is autistic and has been in crisis, was given an appointment 2 weeks away. The receptionist was very specific about the date and time. However, when we arrived, we were told that his app was for the day before, which was NOT the date given to us. We live in Amado, so it was over 30 miles to get there. They made another app for several weeks away. I don't trust them now. This is no way to treat those in need. I will keep this app, unless I find another facility to take him to that is more reliable."

"On a regular basis I make appointments for my son and we go from Vail Arizona all the way to St Mary's to have them tell me he didn't have l an appointment for my son on the schedule so it's wasted time and money which is difficult given the gas prices there are several therapists and doctors that leave because they're not happy there they're treated just as bad as the patients are if you're looking for a place I would suggest not to go there every patient there is a paycheck and that is it. We will be going to another facility I've had enough of these people"

"I was sent here while in crisis. I waited, like an anxiety filled fool, for 5 days for them to have a "Case Manager" call me. I left 3 messages with the Intake Coordinator, who assured me that if I needed to talk to anyone before I hear from the CM she'd be there. I never got a reply to a single voicemail. My last message was to tell them to deactivate any services they had started on my behalf. I will not continue forward with another office that discards me before we even start."

"Incompetent front desk staff. I was under a time sensitive court order to get a mental health evaluation. I made an appointment, then on the day of the appointment was told ”you’re not in the system.” So I made another appointment. The individual took down my name and address and phone number. I called back a week later to confirm the appointment and was told ONCE AGAIN ”you’re no in the system.”"

"I worked at CHA for 6 months. In the time I was there on the mobile crisis team I realized that CHA only cares about money. If you want to genuinely help others or need help from a crisis team, CHA will provide very little as their techs are only allowed to assess and assist with the petitioning of patients. Monicia Hill is the manager over the ceisis team. Mo has been known to request bribes from her employees in the form of gifts at christmas parties in return forvery little favor. A bribe is unnecessary to begin with. If you consult her for issues in the office she will cover them up if the person you reported is her friend or has accepted a bribe. Monicia fired me because I reported her friend and coworker Cynthia Zahid for harrasment, which Mo Covered up. I found this out later though her Supervisor Paige who informed me that nothing had been done about my report. The fact is I was never given a 90 day evaluation that i was always told was coming. I was never given any training though I do have 13 years in Behavioral Health. And they refused to give me a partner for safety. In the end they said i was being fired because i argued with Nursewise over the phone about my getting disproportionate amount of calls. Another manager there named Anthony Trumell Smiley makes his female employees uncomfortable by speaking of women in regards to his romantic exploits. I have suggested multiple times for my coworkers to report to HR. But they are worried about losing their jobs due to her covering for Anthony as Mo has done for Cynthia in the past. If you do not play the role of the supplicant and look up to them; if you question them in any way you will be seen as a threat. And Mo gets nervous when she is around people that are mentally stronger and better educated. Cynthia was was a manager and now works as a tech. Her partners always complain behind her back about her work ethic and her poor treatment of patients. And Mo says that she "just can't see" how her employees are unhappy. They are too worried about losing their job to a tyrant. As a strong Man of Taino and African descent I can most assuredly say their crisis team is a snake pit with "Uncle Tom" leadership. Power to real black people."
