Blackstone Community Health Care School Based Health Clinic is a federally qualified Patient Centered Medical Home located at 24 Summer Street in Central Falls, Rhode Island. Our clinic offers a wide range of affordable health services, including pediatrics, behavioral health, internal medicine, family medicine, obstetrical and gynecological services, dental care, optometry, and more. As a culturally diverse health center, our goal is to provide high-quality, low-cost healthcare to the community. All patients and visitors are required to wear masks and will be screened upon check-in. We are currently accepting new primary care patients for all services, and our Express Health walk-in service is available for same-day sick appointments in Central Falls.
