We are dedicated to bringing skilled nursing and therapy services directly to your home, helping adults and seniors recover from surgeries, illnesses, and injuries, while also managing chronic medical conditions. Our Short-term Rehab Services are designed to prevent hospital stays and are prescribed by a physician to ensure personalized care. Our team of specialized clinicians, including registered nurses (RNs), licensed practical nurses (LPNs), physical therapists (PTs), occupational therapists (OTs), and speech-language pathologists (SLPs), provide approximately one-hour home visits. These visits continue until your recovery goals are achieved, with the support of medical social workers and home health aides as needed. Typically, episodes of care last between 30 and 60 days, with the option for renewal if necessary. Most importantly, our services are generally covered 100 percent by Medicare or private insurance.
