"I’m so appalled and concerned by the care or lack of that was given to my grandmother by people I use to work side by side with, ones I looked up to at one point. It’s hard for me to comprehend the lack of accountability(letting a CNA leave my grandmother unattended in a vehicle with the motor off while in Zaxby’s for 20+ minutes), compassion for others(instead of assistance with feeding they claimed my grandmother was combative so they just didn’t feed her, she lost down to 100 pounds. At her new facility the first week she gained 1 pound and now 4 weeks in has gained 6 pounds), and disrespect this facility’s staff has for their patients(the nurses smoking vapes at the nurses station with a patient next to them on oxygen, the administrator walking the halls beside patients puffing away on her vape). When a person accepts a management position you have to differentiate the friendships you’ve made with other employees because they are now your subordinates in order to maintain accountability in the workplace. There is none of that in this facility. If medical errors are made, management will lie to the families face, delete grievances/cover up for Nurses and CNAs under them, instead of taking corrective action to prevent these errors from occurring again(a tourniquet was left on my grandmothers arm for over 48 hours admin assured a grievance would be filed and an investigation would take place. There is no said documentation in my grandmothers medical record other than our families documentation that we took ourselves of this incident, no ultrasound, no venous Doppler, no doctor visit, NOTHING besides removal of the tourniquet. There could be permanent damage to her arm but nothing was done to ensure that her health was not compromised by this medical error. There was no teachable or corrective action, NOTHING, no effort of NOTHING. It is extremely sad to see what was once a great place for the elderly turn into a family’s worst nightmare. The majority of the positive reviews are from employees, their family, or affiliates of the facility. I personally worked at this facility years ago & was proud to say that but recently was mortified to see what it has become today. 1. Deana Caudill is the wife of an employee 2. Yorlanda Peterson is an Assistant Director of Nursing at the facility 3. Lori Boland worked at the facility 4. Brittany Caudill works at the facility 5. Brad Nobles was the former administrator 6. "My mom was in this place for a few years. We were in and out every week to check on her. Her room was dirty and she smelled and would be in soiled clothes many times. Lots of times we would get there and they would be of dressed her in her pjs instead of her clothes which my sisters washed and took to her every week. She was on a liquid/soft diet and was supposed to have help eating and drinking it at every meal. No one ever while we were there offered to assist her, they just assumed we would do it, which we did. She lost so much weight, hospice nurse was called in to check on her regularly. She walked in one day to find our mama was trying to feed herself with her hands her liquid diet because NO ONE had done their job and feed her. Other words, our mama was losing weight because no one was feeding her and she was starving! When her clothes are washed, there are lots of food/ liquids all over them. So much so, they can hardly be cleaned. She has plenty of bibs but no one uses them or her clothes would not look like that. So the weight loss, the nurse seeing what happened with no one feeding her and no one offering while family is there, her clothes in a mess with food, makes us think she is NOT getting the nutrition she needs. We don’t want our mother living like this and you shouldn’t either! I urge you to check often on your loved ones there and what you think you see is not always the case! Document all visits and ask for copies of everything involving your relatives. This is not a good place.."
"My mother was in this facility for over two years & the care received there was totally unacceptable. So many things happened there I couldn’t possibly list them. Just a couple of examples: she was left in a transport van alone in cold weather while the attendant went inside a restaurant for lunch; a tourniquet was left on her arm for 2 1/2 days after a blood draw, leaving her hand swollen & sore. I was there every week & have never seen so many uncaring people in a job that needs someone to have compassion for the residents. My mother was totally at their mercy as she has Alzheimer’s. If you care about your loved one, you don’t want to have them in this facility. So thankful to finally have her in another place where people actually take care of her as they should. I would give them zero stars if possible. And no, I don’t want to discuss this further, we spoke with admin & others numerous times to no avail."
"Treated my mother like trash, like she was gonna die and they were just dealing with her!!!! It was like jail, concrete walls and a thin bed, food was terrible!!!! My mom just needed a little help getting on her feet after an illness. Me and her are way better humans than the nurses there, we have helped our community tremendously... I just wanted her to have a little more respect, and I believe they charge like 12,000 a month to be there, it would be hard for me to stay there if they paid me!!!"
"My mom was here for 10 days last month after neck surgery that left her needing rehab. The building itself is a little older It's not a new facility, And we had one issue with the bed not being stationary. However what impressed me so much was the staff and how they treated the patients. As family we were welcomed to be there as much as we liked and never felt in the way or like they were too busy to listen to my concerns. From the moment we got there Yolanda came out to meet us and truly made me feel like they were happy my mom was there so they could care for her. I'm sure they were short staffed like everyone else is but I felt comforted while I wasn't there knowing The care she was getting. I would recommend this place for anyone wanting to make sure the people taking care of your family are kind-hearted and truly care about the patients. They only other thing I would mention is we didn't get quite enough time of rehab or occupational therapy, however I just assumed this was due to staffing and wasn't something in their control. The therapist she did see were very good. Thank you Signature Healthcare!"
