Comprehensive Support Services:
- Education and Advocacy: We educate patients and families about comfort care and symptom management, assist with healthcare and insurance questions, and advocate on their behalf.
- Collaborative Care Planning: Our team works closely with physicians, nursing staff, and families to develop and implement individualized care plans.
- Personal Care Assistance: Regular visits provide personal care, support for RNs and LPNs, and help with light housework.
- Spiritual Support: We offer support to patients of all denominations and welcome care from the patient’s own clergy.
- Volunteer Support: Specially-trained volunteers offer respite for families, providing companionship, light cooking, music, and creating comfort blankets and memory quilts.
Respite and Continuous Care:
Understanding the physical and emotional stress on family caregivers, we offer short-term respite stays in our peaceful inpatient locations, adhering to Medicare guidelines. In crisis situations, continuous care is available, providing the prolonged presence of a hospice nurse or aide in the patient’s home.
Volunteer Opportunities:
We welcome and appreciate volunteers with diverse skills—from gardeners and hairstylists to musicians and writers. Volunteers must complete an application and receive training based on their area of interest. They can choose the amount of time they wish to volunteer and the geographical areas they wish to serve.
Historical Roots:
Our organization’s roots date back to the late 1970s, when Rev. Dr. J. E. Victor Carlson founded Hospice Care, Inc. in Greene County after his wife Ruby’s terminal illness. His vision led to the creation of one of the first hospice programs in the state, which eventually merged with The Washington Hospital in 1996. The Donnell House, opened in 2002, remains the only inpatient hospice of its kind in Washington County, thanks to generous community support.

"A friend just recently passed away and we were not allowed to visit! The man was dying at a very young age and was no longer in contact with his family all he had was his friends that tried to make that last bit of his life enjoyable! He passed away alone and they should be shameful of that! Will not recommend friends or family to go there!"
"As a hospice nurse myself previously, I had very high expectations for care for my FIL during his final days. Had it not been for the on call supervisor Amber and the wonderful nurses at Paramount Peters Twp my FIL would not have had any quality care. I was truly disappointed and thank God I was present for my FIL to quickly manage his symptoms and have him pass as peaceful as possible. Will not recommend Concordia Hospice unfortunately."

"My grandmother had a short stay here in 2017. While she was only at the Donnell House 1 day the comfort and kindness shown was spectacular. My grandmother had passed prior to family arrival. My great grandmother had drove from Ohio to “visit” her not knowing the seriousness of the matter. My great grandmother suffered from severe memory issues and in stressful situations forgot what she was just told and need told again. When my great grandmother saw my grandmother had passed she needed the news broke to her gently, and a multitude of 10 times at least. The aid (Jamie) held her hand and continued to walk her through it and assured her the passing was peaceful until she finally coped with the matter. We were able to goodbye in a comfortable setting and the care was 10/10. Would definitely use as my hospice of choice again."
"We brought Concordia in as hospice provider for my 94 yr old father about 6 months ago. Their service, in the beginning was a 4-5 star. In the past 2+ months it has fallen to an unacceptable level. They communication with my sister, Dad’s Medical POA, has become her chasing them for information as well as the assisted living staff of where Dad resides. I strongly recommend you look else where for services for your loved one. I DO NOT recommend Concordia."
