Dr. talked to me about the five wishes and left the booklet for me to fill out and return to him or another nurse practioner at my next visit in April."
"I love the fact that they call and check up on you. If you’ve made a visit to the hospital they got word you were sick or not feeling well they talk to you about your medicines. They talk to you about your daily activity. They talk to you about if anything is bothering you is there anyway they can help you with anything and they listen they listen."
