She said the doctor was friendly, courteous, made her feel very comfortable. The office staff was very nice as well. She was excited to have been referred to this doctor."

One suggestion is to have the electronic required history sent to me done at least 48 hours ahead and not after 7 pm the night before a 10am appointment. It took me 40 minutes to complete. I easily could have not seen this and not had enough time to complete it. I would much rather. Do this lengthy electronic required history at home and not sit in the office."

"I found Dr. Kozak to be warm and helpful in dealing with my symptoms, while moving a bit quicker (just minutes) than I would prefer (knowing that many in the industry are pressed to spend little time with each patient). I would have liked for the assistant who brought me into the dr's room to have been a bit warmer and a more attentive listener.
