"This is a horrible company! They double charged me in September for a "mistake" they tried to blame on my surgeon and I'm still waiting for that refund more than two months later. I've called several times and just get the run around. Nobody seems to be able to do anything about this. They tell me "it's up to accounting." If this isn't resolved by the end of next week I will report them for fraud. Be aware--they don't take insurance of any kind (because it's "too much trouble"). They aren't accredited by the BBB. They are involved in legal disputes as an unregulated formulary."
