We had an issue with how Lansinoh branded their products which meant a lot of back and forth over the course of a couple of weeks and the PE team saw me through to be 110% sure I got the correct pump I’d ordered. "This provider charged us over and above in-network rates. They claim it is for an "upgrade" which is their excuse to bill you above contracted rates they are bound to accept. In network providers cannot ask members to pay the overage on products or services, that's what it means to be in network, that you'll accept the agreed rate. My insurance paid full price for the pump. We were then charged an "upgrade" fee so they could get around accepting what insurance paid. They bill insurance for the max they can, for a product they don't provide like a "basic pump" and if it's not enough, will bill you the rest. They will try to get your permission to do this upfront, doesn't make it right in my opinion. Recommend going elsewhere for what you need."
