"Horrible experience with this office has been a rough go since day 1 only went there because my other office closed and they have been a nightmare to deal with and now is charging $40 to fill a form out when I just payed them to fill the same form out a week prior and had another piece of paper come in the mail for that form, place is a scam and my whole family is pulling from the office now, ridiculous that a doctor office tells me the doctor doesn't get paid to fill forms but can take 1000$ to take my temp when I come in gotta love it.
