"Avoid this office.. The staff treated me horribly. Been a patient for over 10 years. Turned 65 last year and there is a welcome to Medicare health screening consisting of a number of tests, screenings and questionnaires, all listed on the Medicare website. During my annual physical, I pointed out the screenings to Dr Cheryl Case Diaz.. She glossed over several of them.. She did not give me the depression screening, kind of important for senior citizens. The next day I called the office and asked them for the screening.. They had no idea what I was talking about.. I went to their office and asked them please to find the test form. No idea. After several more phone calls they found the test and told me I had already taken the Depression screening... Turns out it was filled out by someone else and was all N/A...Wonder who did that? Asked to send me a form to fill out.. After a month of this they finally sent me a blank form.. I filled it out and sent it back to them... They never got back to me, even after I asked them repeatedly to give me the results, which i knew were not good. Then they started forwarding my calls to phones that no one answered, no returned calls. IMO they are either ageist or racist. .Either way they are a horror show of medical "professionals." Avoid."
