"If I could leave zero stars I would ! By far the worst experience in a doctors office I have ever had ! I have never in my life experienced anything like this Dr Faulkner Was very rude and nasty ,she was very argumentative the whole visit ..she did not want to listen to anything I had to say she talked over me as I was explaining things to her ! she entered the room and did not greet me at all instead she stormed in the room and stated told you that I cannot help you ! You will need to see an endocrinologist, your pcp is “doctor Henry” I don’t know why you are here . We have our own set of patients in this office I then explained to her that I was having a very hard time finding someone and that the pain that I was having was getting worse and not better ! I then begin to ask her for some pain meds for my pain and discomfort, instead she just assumed that I was talking about medication for my thyroid,, etc“ she stated I cannot give you medication “ my blood pressure was on the low side but she never mentioned that ! ,she was just rude overall , I do not recommend this doctor at all , very bad experience with her I left very upset and crying .after leaving I did express my situation to a manger, nice guy by the name of Brandon he was very helpful ! I pray that no one will never have the bad experience I had I will never go to this location ever again"

"Christopher Dziopala NP Is an extremely rude healthcare provider. The rudest I have ever encountered at this facility. He stood across the room from me, didn’t exam the lump in my breast, which was my chief complaint. Matter fact, stood by the door the entire 3 minutes he was in the room. Wrote me a referral for an ultrasound that when I went to the appointment, was $250 dollars, that I could not afford to pay. The woman behind the desk told me that Evara usually sends the patient with a paper stating a discounted fee was paid in their office.. along with bloodwork orders which were another $50 dollars and their office staff said the exact same thing!! I made another appointment with a different nurse practitioner and they not only examined my lump but also helped me with the discounted rates for my tests that should have been provided in the first place, but were NEVER even mentioned!! That $250 ultrasound, I paid $25 dollars, the $50 dollar bloodwork, I paid $10 dollars for right there, in the Evara office at the front desk. Mr. Dziopala did not care about any of my issue’s or my ability to pay. I will NEVER waste my time with that man ever again."

"Dr Faulkner is kind and understands my concerns. It is difficult to get a hold of the phone but that’s the case for most medical offices. The office always gets my insurance authorizations done quickly after being requested"

"This review is regarding Christopher Dziopala NP at this office: After a 45-minute commute with my 2-year-old toddler, he refused to see me because I was a few minutes late. I did not anticipate bad traffic or the confusing hospital layout. I arrived at the hospital at 9:05 a.m., and my appointment was at 9:15 a.m. and I completed all the pre-check-in questionnaires online at home before going there. They are located in building 4 when I walked into building 1 with my stroller, so it was just walking to find their office delay. I was desperate for a refill of my heart medication, and it was the last sick day I had this year from work. I needed to see any doctor, so I took the earliest appointment possible that fit my work schedule. When I pleaded, they asked me to wait for a vacancy, but that the vacancy was not guaranteed. There was 1 person in the waiting area that arrived after me, so it was obviously not an exceptionally busy day for them. I have waited 60 minutes in the waiting room to see my cardiologist every 6 months for the past 3 years, but I guess being a few minutes late is intolerable when it’s the other way around. I am usually suspicious of sob stories, but this actually just happened to me. Not to mention today is my toddler’s second birthday—what a waste of gas and mileage. Definitely never going to this location or this nurse ever again in my life!!! New patients; avoid them if you can..."
