"This review is more for his staff and not the physicians. Called in to request my medical records for my own personal records since I relocated to a different state. The first time I called I was told that there was a fee for this (there is no fee for medical records unless its a disc), also that I needed to fax over a request realize form. The funny things is that when I told them that this was not accurate information and I knew this for a fact since I'm also in the medical field, only after this was I told that all I needed was to fax over a note stating that I needed my records which I did, not being this enough once again I was told that this was not sufficient enough and that I needed to fax another note explaining that this was a transfer of care. I will make sure I let my referring doctor aware of this and I really hope that they are not charging people for medical records THAT WE ARE ENTITLED TO."
