"They ask for one full business day to complete physical paperwork. 2 weeks later paperwork still isn’t completed. As she sat in front of me and completed for all 3 kids it took less than 5 minutes to complete. Which shows not busy...but lazy. When the practice took over doctors came and went. Downhill since Dr Collins retired. Terrible. Started the process of changing doctors for all 5 kids."
