"I saw Dr. Racek last year for the first and last time. She was very nice and asked if I minded if an intern observed. I agreed to allow them to observe. However, the intern did the complete exam-which I never agreed to, and then submitted her notes as an annual well visit instead of an annual well woman visit which caused my insurance to reject the claim for my annual physical with my primary care physician. I have tried for over one year now to get them to fix it and they refuse to admit any wrong doing. Seems like insurance fraud to me!"
