I receive a form in the mail for "Release of Health Information" but have no one to send it to. I'm left scrambling to find someone else. I would expect better from your practice. This is just not fair - I'm left without a PCP and no help in finding another. Is this age-related? Because I'm on Medicare? Completely negligent by this practice to your patients.
