To determine if you require a home diagnostic sleep study, we invite you to complete our sleep questionnaire. This questionnaire assesses your sleepiness and evaluates common risks and symptoms related to sleep issues.
If you believe you are at risk of sleep apnea, please download our referral and consult with your GP. You can also find a list of Telehealth providers on our website.
Once you have a signed referral from your GP, you are ready to schedule your sleep study. Simply email your referral to sleepstudy@airliquide.com. Please note that a signed referral from your GP or medical specialist is required for scheduling a sleep study.

"Issue was resolved a few days later after contacting Air Liquide management. I recently bought a nasal mask from Air Liquide at Morphett Vale. I didn't know that the business had a non-refundable policy and the sales person did not inform me that you cannot return the item after purchasing. When I arrived back after 15 minutes with my sealed, unopened item wanting to return it, the sales person informed me that the store has a "refund policy" stuck near the bottom of the counter that states... "we do not *normally* refund if you have simply changed your mind, made a wrong decision or been incorrectly prescribed". "Normally" to me means that usually we do not, but on occasions we might. Therefore, this policy contradicts itself. Furthermore, the sales person should be enforcing the policy before the transaction. I was surprised that even though the item was sealed and had only left the store for 15 minutes I was not entitled for a refund. This is poor business practice. However, if I was aware of this policy in the first place I would not have bought an item from here as I was honest with why I was returning it. I am not happy with this service and I would not recommend people to purchase from here, as I believe you're entitled to return an item that is still sealed and had only left the store for 15 minutes! I have never written a bad review before but this honestly made me very unhappy with how this business handles its sales. If you have a policy like this I believe it is the customers right to be aware of this kind of policy instead of being well below eye level for the customer (as shown in image)."
