"I placed my brother there. He was never physically abuse but because he could not walk and needed more help than most there, they neglected him on that part. Adult protective services were called after 2nd hospitalization. He ended up in hospital twice for dehydration because he wasn't provided water. They barely feed him enough food. I showed up to visit, he had blood spatter and droplets all over his clothes and smelled. Another time he was in lounge. He had a sandwich but it was not on a paper plate, just on the table. My brother was rushed to hospital 8-29-23, 11 am because he had a fever and wasn't acting right. They said as soon as they seen him they called ambulance. I got to the hospital and these pictures are what I took. He had fever of 202.8 and he would open his eyes and they would veer off to the side. They did blood work and head CT, no stroke or bleed. He was placed in ICU. He was suffering from a lung infection and heart, liver and kidneys were shutting down due to dehydration. At this point he was unresponsive and in a coma. By 8 pm, I removed all life support and nurses made him comfortable. My brother was at Collinsville nursing home on summit from January to August 29, 2023. He died Aug 30th, 2023 at 1:05 pm. at Anderson hospital in Maryville, IL. I went back to nursing home to retrieve his bear and it was gone. Even though staff was nice, I regret placing my brother there."

"My father was recently diagnosed with early onset dementia, and we were desperately looking for a place that was close to my mom’s house . DaVita, the Director of social services for both locations. Lebanon and Collinsville worked with me throughout the weekend was patient and kind and understanding . From the staff upon answering the phones all the way to speaking with her were everything and kind. She called to follow up several times on his behalf. Her kindness reflected throughout the staff, and if that’s any indication of a good place that’s the place that I would want to send my family."

"Please don't put your loved ones here! This place is the bottom of the barrel and so is half of the staff. It is filthy and they don't clean. Not even between the last resident leaving one room to your loved ones being admitted. The staff is rude until they find out that you are a Registered Nurse and can legitimately cause havoc. The good reviews are from employees!"

"My brother lived here for 2 years. The administration is very unprofessional. There was peep holes in the public restroom. I read an article from BND stating a nurse didn't apply CPR to a patient so she died. They also received major infractions because their fire doors and sprinkler systems didn't work. The food was terrible. They do have some very good employees...not all. The administration is terrible."
