Our Services

At Success Community Care Clinic, we begin your journey with an initial screening and assessment conducted by a licensed therapist. This thorough evaluation helps determine the most appropriate path forward for your treatment goals. You will receive up to three initial screening visits to ensure a complete understanding of your needs.

Following the assessment, you will meet with one of our prescribers to develop a personalized treatment plan. If required, medication can be prescribed, and we ensure all clients are fully informed about their medication options and expected outcomes.

We offer both verbal therapy and group therapy sessions. Our flexible scheduling includes options for telehealth and in-person appointments to accommodate your needs. Group therapy, available both virtually and in person, provides valuable peer support and communication.

For those requiring additional support, our clinic connects clients to appropriate programs and facilities, leveraging our extensive resources to ensure comprehensive care. Our Mobile Medical Support Team (MMST) is dedicated to assisting individuals in unstable housing situations, providing essential primary and preventative care.

Specialized Support

- Substance Use Therapy: Our Certified Alcohol and Substance Use Counselor (CASAC) offers individual, family, and group therapy, supporting you on your road to recovery, including harm reduction strategies.

- Vocational Rehabilitation: Our Rehabilitation Counselor assists with employment-related services, including resume creation, skill-building, and ongoing support, whether at home, the office, or your job site.

- Case Management: Our case managers are committed to addressing your needs, providing referrals to community resources, employment assistance, housing aid, and more, meeting with you at home, in the clinic, or in the community.

- Peer Support: Certified peer specialists offer individual, group, and family support, helping you restore your well-being and build life skills in various settings.
