Dr. Mohammed R. Azaz, MD

"Dr azaz is no longer in the office you see everyone but him. Twice they changed my appointment time without my authorization the day before my already scheduled for months appointment. Then when asked about it proceeded to tell me they did it for their own reasons and process. Why have a patient pick their own time if you can't accommodate. And to change without enough notice is unacceptable."

"TL;DR: Dr. Dacosta is no longer with this practice I called to make a follow up appointment at the Ronkonkoma location after an emergency room visit and was told they didn't have a schedule for Dr. Dacosta right now. I asked what that meant and if it was 2 or 3 weeks out and was given a vague answer. I wound up making an appointment with Dr. Azaz thinking at least Dr. Dacosta was still there and could look over my records if needed. I hung up the phone with an odd feeling and googled it. After some digging I was lucky to find an article welcoming her to her new practice. I am so disappointed in the way this was handled. I followed Dr. Dacosta from when she was in Medford. I trust her with my health and I think it's very difficult to find a doctor who cares and is so thorough. I wasn't even originally a patient of Dr. Azaz. I followed Dr. Dacosta to his office and should have at least been told that she is no longer practicing there even if they couldn't tell me where she went. When I called back to cancel the appointment there were no apologies. They tried to get me to see the nurse practitioner instead. I don't care if this sounds dramatic, but this is about my health. I can't even imagine what would have happened if I was in the middle of a serious issue. I'm sure Dr. Azaz is a good doctor, but he doesn't get to decide who I see. Hopefully, he will take this into consideration for any future partner splits. Patients need to come first."

"Doctor is great it’s the unprofessional nurse staff that cause me to let go of this pcp and leave this 1 Star. I called over 3 weeks left 12 voicemails for my ultra sound results with no call backs not even to say everything is okay. I had a nurse from an unknown number call and say she was with the office and only stated she has called me but of course that odd voicemail was untrue which concerns me that a nurse will lie for whatever reason. She also never mention my results that the doctor had me get done so I’m not sure if that was even a nurse or just an office employee."

"You will have a wait. It's because Dr. Azaz takes the time to listen to patients and care how they are doing. One nurse a a new PA didn't hesitate to take the time when I needed unexpected help, last month. It was 30-45 minutes out of their schedule, as I was there for a simple sonogram. The woman doing the sonogram took the time, as another appointment cancelled, to do my sonogram after my unexpected emergency. He cares and talks with your other doctors. Actually talks to & coordinates with them."

"Have had 2 appointments with Dr Azaz. First week I was in severe pain. He sent me for a stat x-Ray. I was notified the next day of my results. Followed up one week later, yesterday, and was still having pain. He did a thorough exam and sent me for a stat CT scan. They called the radiologist's office to get me an immediate appointment. I am very pleased with the care I have received. My only complaint is that once you are in the exam room it is a long wait before you see the doctor."
