"Dr. Guarlnick authorized for new cpap machine. Nurse sent this directly to cpap machine vendor Apria who shares their office. My insurance and I tried to get a copy of the authorization but the nurse said they do not do that. After many hassles, got a copy of prescription that I can take to other place. Sounds like an insurance fraud or shady business arrangement."
