ISU Health Center, also known as the UAP Clinic, is located at 567 North 5th Street in Terre Haute, Indiana. This institution provides high-quality healthcare services to students and employees of Indiana State University. All undergraduate and graduate students, as well as current employees taking one or more credit hours at ISU, are eligible to receive care at the Health Center. Patients must present a valid University ID card upon arrival.

The Health Center accepts private insurance and participates in many insurance plans. Patients should bring their insurance card along with their University ID card, and co-payments are expected at the time of service. Unpaid balances will be sent to the guarantor listed. Payment can be made with the Associated Insurance Plan, private insurance, cash, check, or credit cards.

For added convenience, the UAP Clinic Convenient is located just two blocks south of campus at 221 S. Sixth Street. Hours of operation are Monday to Friday from 5 pm to 8 pm, Saturday from 9 am to 3 pm, and Sunday from 1 pm to 5 pm.

Fees can be billed to health insurance, and no co-pay is required for injections only. Immunizations are available without an appointment between 8:30-11:00 am and 1:00-3:30 pm. Some immunizations are required by the State of Indiana, and new student immunization forms must be returned to the Office of Registration and Records.

During the summer, the Health Center operates Monday to Thursday from 8 am to 3:30 pm and Friday from 8 am to 12:30 pm, with a lunch break from 12-12:30 pm Monday to Thursday. For more information and required forms, visit the immunization requirements webpage.
