"From the beginning late 2016 through 2022 everything was good, my Mom surrounded a good reception desk & nursing staffs, Until 2023, everything has changed worse everyone who has hired, every employee were usless. And treating my mom disrespectful, especially the Philippines Small Lady who has been very cold hearted since she started working. Today she started disrespecting against my Mom. And I had to make a report against her. But I told them if this phillppine lady ever do this against My Mom?? I told them that will make a police report as harassment against my Mom after her behavior like this. .If she ever do this again. Anyway, we'll keeps eye on her and her type behavior"

"First let me start by saying I'm on to leave good reviews, and take bad reviews with a grain of salt. Afterall, most people only speak up when dissatisfied. However, this review needs placed and what we've experienced here can't go unspoken for. This is my first stop- I'll be making a few calls. My father is in the late stage of dementia. The only thing that qualifies this as an alzheimers facility is having a pass code to get out the door. The people here are barely qualified for direct care let alone cognitive care. On one visit I was they for two hours and no one checked on him. My stepmother stopped by on another day during his repit stay only to find him butt naked and cold, laying on WET sheets. His aid was nowhere to be found. The next day he was going home by ambulet and Genesis of Perrysburg sent him home in a diaper, t-shirt, and socks. No pants, no jacket. Thin hospital blanket. In mid November. Their excuse was they didn't know he was going home? Well they knew they put him on thr Gurney half naked; nevermind it would taken less than five minutes to dress him proper. This place is without compassion or understanding of dignity. The fortis stand behind a desk and talk, in the dining room they complain about how horrible their job is in front of residents.. I noticed how poorly they rate in US News nursing hint report. This place shouldn't even be open. It's disgraceful."

"I wouldn't want my worst enemies loved one sent here. They do NOT have anyone's best interest at heart except for their pocket book. They are under staffed. They hope to shove patients in a room and forget they are there. They never followed through on any of their promises. Morning medication does not get distributed until 3 pm while evening at 5 pm. The only thing this facility could accomplish in a timely manner was meals if you could call it that. Most meals I witnessed did not look appetizing at all. My mother was there as a short term patient to recover and she would have to wait 2 to 3 hours after putting her light on for anyone to come see what she needs. I would hate to think she actually would be experiencing a medical emergency and no one would care. Due to their lack of care and concern my mother has been back to ICU multiple times. The last trip to ICU was literally 2 days after returning to their facility from the hospital. They refused to give her medication that might create more work for themselves. Diuretics prescribed were never dispensed and it was too much to ask them to keep her bipap machine accessible and connected to oxygen so she can keep her CO2 levels in check."

"I don’t even know where to start. Genesis of Perrysburg was repulsive and nauseating, even just thinking about it now makes my heart ache for the residents and makes my stomach churn. There was such an odd odor as soon as I walked in, the best way to describe it was a rotten moth ball (if that’s possible). The smell got worse as we went in closer to the resident’s rooms, it began to smell just flat out foul. There was no staff member at the front desk, nor was there a way to call out to anyone when I went in for the tour. I waited for over 8 minutes before I was able to begin the tour, and I went at 3pm on a Friday so I wasn’t expecting them to be busy (they weren’t). The facility was outdated, unkempt, and there were many cosmetic damages done to the walls and floors. There were small pieces of trash everywhere in the facility, ranging from wrappers, to scraps of paper, to what looked like the ends of cut off hospital bands as well as a used bandage. The paint was peeling and cracking, the wallpaper was sun bleached as well as the linens, the tile floors were stained yellow and brown in spots and were rusting, and the ceiling had a lot of water damaged areas. The furniture was just as outdated, and was clearly falling apart and many marks on it. The building has definitely seen better days. The Executive Director, Erin, had told me that the aides each had 12 residents, all with varying levels of care. The rooms are shared, with one very small restroom, which I know would be incredibly challenging to get residents into (being a Resident Assistant myself at another facility) especially if the resident should need a lift. Each room smelled exactly the same as the halls did. The lighting was awful, it was so dark inside the facility. I was taken to what was a memory care unit, and the smells and appearance only got worse. From there, I was taken to a small courtyard. The area was small, and this was also where I spent a majority of the tour. Erin seemed to be focusing on the outdoors (which was nothing more than a small side yard with two garden boxes and a bench), and was not spending much time showing me the inside of the facility. To me, she knows the condition that her facility is in, and it felt like she was attempting to rush me through the tour so I couldn’t actually ‘see’ the facility. The patients seemed depressed. There were two women who particularly stood out to me. The first woman I saw was sitting at the nurses station, with her head slumped low, and she did not smell good at all. Erin, had addressed her by name and said hello, and when the woman responded she sounded incredibly sad. I only encountered one STNA while I was there, and she looked like she hated her job, and looked like she was ready to go home. I would not want to approach her and ask her for help. As for Erin, she did say hello to each and every resident we passed by, and greeted them by their name which is nice to know that she knows the residents by their name. Genesis is nowhere near a standard of satisfactory, but it should certainly be used as a standard for how not to run a nursing home. The only positive aspect of the facility was the rehabilitation gym, which I was not even taken to see! I’m making this statement solely based on me passing by the gym twice while in the facility. I would never, ever place my parents or anyone I know in Genesis. There are far too many problems with the facility for them to be fixed. Based on the status of the building, the attitude of the employees as well as the residents, I would be willing to bet that the company has an awful ‘foundation’. It seems to me that the company is just that, a company. It doesn’t appear to me that they care about their employees nor their residents at their facilities. Having said this, I would like to make it clear that I believe that Erin and the one STNA I encountered was doing the best they could giving their situation."

"Facility does not provide resident driven care. Residents quality of life and safety are put second. Any concerns or suggestions to improve resident safety and quality of life are met with resistance, poor excuses, inappropriate interventions that diminish the residents quality of life or have the potential to negatively impact residents safety. This facility should be used as an example of how NOT to run a nursing home. Genesis Health Care needs to get out of the nursing home industry entirely."
