Exclusively open to Horizon House Program participants and staff members, patients at FPCC-HH have access to a wide range of care services including Dental, Pediatrics, Health Education, and Social Services. Patients can also access the Patient Portal for scheduling and download a Patient Information brochure in Spanish.
