Our facility provides both in-person and Televisits, ensuring flexible healthcare options to suit your needs. We are open Monday through Thursday from 8:00 am to 8:00 pm, and on Fridays from 8:00 am to 9:00 am, then from 10:00 am to 5:00 pm. For your convenience, we offer free parking next to our building, although spaces may be limited during peak hours. Our main entrance is on Revolution Drive, directly across from Home Depot, and we are accessible via the Orange Line at Sullivan and Assembly Stations.
To streamline your visit, we encourage patients with a MyCHArt account to utilize eCheck-in, allowing you to review and update your information prior to arrival. This not only makes your visit safer but also saves time at registration. We provide multilingual support with an eCheck-in User Guide available in English, Haitian Creole, Portuguese, and Spanish.
We understand the importance of having a support person during your visit, so patients are permitted to bring at least one companion to clinic visits, Urgent Care, or the Emergency Department, subject to provider discretion. For those visiting hospitalized patients, please refer to our Visitor Policy page for the latest guidelines.
Our interpreter services are available at no cost if you prefer to communicate in a different language during your visit. Simply inform your care team ahead of time, and our interpreter services will be arranged to assist you.
For non-urgent health questions, you can conveniently send a note via MyCHArt. For billing inquiries, please contact our Customer Service Billing Department at 617-665-3200. Financial assistance is available through our CHA Financial Assistance team at 617-665-1100.
Manage your healthcare effortlessly with MyCHArt, our secure online system that lets you request appointments, refill prescriptions, and review lab results from any device. For prescription refills, contact your pharmacy directly, and if you run out of medication, inform your pharmacist for immediate assistance.
We offer same-day or next-day Virtual Sick Visits for certain symptoms through MyCHArt, providing you with access to medical care from the comfort of your home. For urgent care needs, visit CHA Somerville Campus Urgent Care or our 24-hour facilities with short wait times at CHA Cambridge Hospital and CHA Everett Hospital.

"Worst experiences ever. These folks will over-bill you til your last penny. Even if they are in-network, don't expect anything to be covered. They are mischievous money-sucking capitalists and they do not approach any concerns in a caring way. I have tried to find something nice to say for 3 years and I am constantly let done. In all seriousness, do not go here."
"Impossible to get someone on the phone. I spent three hours and a dozen calls. Every time they hang up on me or transfer me back to the opening menu. The last six times I told the operator exactly what had happened and followed their instructions exactly. Finally the last one admitted that sometimes this happens and I must go in person to make an appointment or talk to someone. If you cannot go in person for all communication, then DO NOT GO HERE. You may literally be unable to get anyone on the phone who can do anything or talk to you about anything. I've gone to various CHA locations and types of providers over the years. My PCP is here. I am an active patient. This is the expected experience. I'm happy with my care. I just think it's egregious to have to go in person for all contact."
"Does anyone want to do their jobs anymore. My goodness in the medical field these people are truly playing the game of just having a job. Do not think that you will have a kind and friendly customer service attitude provided when calling this facility. You better visit the location the day before just to prepare for your experience because you'll need to do it twice. Once to anger you thr second to try again and see if you get it right"
"I’d give no stars if that was an option. They constantly move my appointments without letting me know because something else pops up for them so they become unavailable. I’ve been trying to get seen my my primary care for over a year and he never has time, I ask to be seen by anyone who’s available then they cancel appointments at the last minute. They book months out and even if it’s an emergency they’re unable to see you. I had a serious health episode 7 months ago and they still haven’t had availability to see me bc of how they reschedule you out of nowhere. When you call for information they are extremely rude. I’m not talking about just a bad attitude, I’m talking about calling you dumb over the phone and laughing at you which is so unprofessional. I can’t explain how much i hate this place."
"Before I leave CHA for good, I would like to tell you the reason why. First, I moved my kid’s healthcare to Lahey Primary Care for the same reason that I will never use CHA ever again. The clinic Union Square Family Health Care at 5 Middlesex Ave, Somerville, MA 02145, has the worst customer service I have ever encountered. I'm not talking about the doctors. I'm talking about the front desk staff, the same people the answer the phone and ignore a serious request. I had a health concern since Wednesday and on Friday I called because I needed my medication. My refill was still under processing at the pharmacy. I called the clinic because this always happens every time I have to refill. I wanted to talk to someone about changing my prescription and the rude under trained front desk staff was not professional at all. Because of people like her that this network is turning into a butcher shop herding cattle for slaughter. I asked to talk to the nurse, the front desk person “said” she was going to place a request. Because the other option she gave me was to wait until next Tuesday (for something that I needed help with two days prior). I never got a call so it’s easy to see that she never placed that request. Their staff lack empathy and bedside manners. What kind of people are CHA hiring? Does CHA even train these people? Their staff ignores the fact that they are dealing with people that need help sometimes right away and who are going through something that affects them negatively. For them to ignore a patient’s needs and to not take a plea seriously shows that this place shouldn’t even be functioning. They don’t treat people with decency. They treat us like animals. And for this reason, I changed my kids’ network. Now I’ll gladly leave too because I have seen that CHA is not a good network. They don’t take patients’ needs seriously. Specifically, this location. I have dealt with them for seventeen years, and for the past five years things just went from bad to worst."
