Our office provides a range of vital services related to birth certificates, including issuing certified copies for births that occurred within Florida, managing amendments, and facilitating adoption and putative father registries. We also offer specialized services such as Apostille certifications for international document authentication and commemorative birth certificates, which are perfect for preserving family heritage.

Services We Offer:

- Certified Birth Certificates: Obtain certified copies by visiting our office, mailing your application, or ordering online via the VitalChek® Network.

- Commemorative Birth Certificates: Beautifully designed certificates for family keepsakes. Note: These are not valid for legal identification.

- Birth Certificates for Stillbirth: Issued under Katherine's Law, these certificates recognize births resulting in stillbirth.

- Putative Father Registry: Allows unmarried biological fathers to assert their rights in adoption proceedings.

- Adoption Registry: Facilitated by the Florida Adoption Reunion Registry (FARR), connecting individuals affected by adoption.

Application Process:

- In-Person: Visit our office at 83 Pondella Road with the required identification, documentation, and payment.

- By Mail: Complete the appropriate application and send it with payment and ID copies to the address on the form.

- Online/By Phone: Use the VitalChek® Network for convenient online or phone orders with a credit card.

Fees and Payment:

- Standard fees apply for each service, with additional charges for expedited processing and online service usage. Payments can be made via business check, money order, or cashier’s check payable to LCHD.
