"We should have known better to put our mother in this place my father broke out it like a escape inmate then we tried to give them the benefit of doubt there was a lot of red flags but our needed rehab so only a few days in her stay there they let this 85 disabled lady fall on the floor thank goodness she didn’t break anything when question was asked we were totally ignored. No justice for the elderly we can’t even find a lawyer to help us please don’t take your loved one there!!!!! And yes if zero was a star that is what they would have gotten!!!"
"11/2015 I went to visit my grandmother today and was told that one of her sisters had taken her out for the day. I asked when she would get back and the staff seemed to not know. I called her sister and she said that they lied to me. That she was in a hospital nearby because they let her dehydrate. She had been there for 4 days! There is no excuse for the nurses to lie to my face!!! Update 11/1/2022 I'm unsure why the company decides to leave a reply or what they were trying to accomplish after 6 years of me posting my original review. I just noticed it had been deleted. Mt grandmother ended up passing away 12/2015, just one month after leaving my original review. I'm not saying Azalea Health & Rehab was at fault but I'm sure her last days were less than comfortable being in that joke of a facility."
"A relative of mine is being neglected and mistreated in this facility, Azalea Rehab and Health, and their solution for helping to resolve it was to charge me with trespassing and banned from the property. I have been lied to by almost everyone in a responsible position, and my relative is being starved because of poor food choices for his condition, denied water or other beverages, and has sores all over his body that the head nurse said was not on his body. My relative has no one now because my family is allowing this place to slowly kill him. It’s disgusting nurses and cowardice from supervision and administration to fix it."
"Accepts hospital transfers for temp. rehab when other facilities will not. bc AG needed the business so badly? Family member here on 2 different occassions for short stays after regular hospital to regain strength and PT. Food portions very small, so you will need to provide snacks. We had a fabulous nurse named Cierra, but she was often overworked on double shifts bc of unreliable coworkers, but her attitude was fantastic. She saved the whole stay. They found some extra clothes to loan. Seem to be underfunded. Undderstaffed. Rather low energy about the place. Cable tv was sketchy, unsafe wires stretched, worked on by a nice guy resident. Clean enough, just dated. Rear entrance is only option at night and is rather dark outside certain halo of light, especially in rain; and parking lot rather scary dark. Illegal ramp/back entrance w/code entry box out of reach if seated in wheelchair; and no designated parking in back for ppl w/disabilities. Place needs a huge infusion of resources and it could be ok. Director passed us several times, never spoke. They contract out their physical and other therapy, so it is decent, but promises made, not honored. Big rooms on the front hall. Some weird thing where we never saw a doctor, and they used a retired doctor's name on medication. Beware"
"Not a happy time when you have to move a loved one BUT staff at Azalea Gardens have made it a good experience and an easy transition. My mom is a firecracker and they have fun with it and allow her to still be herself. Nurses/aids are super helpful and forthcoming with my mom's care. My mother is happy and I'm just glad she is safe and cared for around the clock!! Thank You!"
