Located at 714 Ridge Road, Room 215, Lackawanna, New York 14218, Lackawanna Vital Records is your local government office dedicated to providing vital records services. Our team, led by Registrar Grace Marrano and Deputy Registrar Michael Bivolcic, is committed to serving the community with efficiency and professionalism.

Services Offered:

- Birth Certificates: We issue certified birth certificates for individuals born at Our Lady of Victory Hospital. A certified birth certificate can only be issued to the person named on the certificate or their parent. The cost is $10.00 per certificate. Out-of-state applicants must pay via money order or cashier’s check.

- Marriage Licenses and Certificates: If you obtained your marriage license in the City of Lackawanna, you can request a certified marriage certificate. The cost is $10.00 per certificate. Marriage licenses are available for $40.00, and both parties must appear in person with proper identification. To avoid delays, please schedule an appointment with the Clerk’s Office in advance.

- Death Certificates: Certified death certificates are available for individuals who passed away at Our Lady of Victory Hospital or within the city limits. These are issued to immediate family members or those with a New York State Court order. The cost is $10.00, with a reduced fee of $5.00 for genealogical purposes.

- Identification Requirements: For all requests, please provide a copy of your driver’s license or two forms of picture identification.

- Payment Methods: We accept cash, checks, and credit cards (with a 2.95% fee). Out-of-state payments must be made via money order or cashier’s check.
