Our team of board-certified and NCQA-certified physicians, along with our skilled nurse practitioners, Melanie Smith, CRNP, and Stacey Coyle, CRNP, are here to address your medical needs with professionalism and courtesy. We understand that medical care and emergencies can be unexpected, which is why we offer appointments for both unexpected illnesses and preventative care to meet your needs promptly.

#### Our Services:

- Outstanding Patient and Preventative Care: We aim to provide exceptional care in a professional and courteous manner.

- Emergency and Non-Urgent Appointments: Patients with emergent issues will be seen immediately or referred to the Emergency Department. Non-urgent appointments are typically scheduled within 3 to 5 days.

- Insurance and Payments: We participate in most major insurance plans. Co-pays and non-covered services are due at the time of service, and we accept checks, credit, and debit cards.

- No Show Fee: Cancellations must be made 24 hours in advance to avoid a no-show fee of $25 for established patients and $50 for new patients. After three cancellations, we reserve the right to discharge you from our practice.

- Medication Refills: For refills during your visit, please bring a list of your current medications. For refill requests, please call our main number and follow the prompts. Note that it can take up to 2 business days to process refill requests.

- Test Results: Test results are not immediately available and turnaround time varies. Normal test results can be viewed online at www.heritagevalley.org/healthlink.
