"Mixed experience. From 2017 thru 2020 I had to be rather persistent with multiple depts & physicians at Kaiser, but eventually obtained treatments that improved my sleep from "awful" to "below average". With regard to the aptly named "sleep apnea dept", there was a 2.5 year insistence on using a CPAP device. This began before Dr Richey arrived at Kaiser, but was continued by her. Despite my insisting to her predecessor & her that I had a nasal airflow issue (not a tongue based OSA). "even during the day I have to breathe thru my mouth at least once each minute" was an important fact that was repeatedly ignored by this dept. I kept insisting to Dr Richey that I tolerated the CPAP mask just fine, that a different mask would NOT help, that use of CPAP didn't help except as a nasal humidifier, that the cpap seemed to cause breathing problems & night awakenings, and that the CPAPs own statistics showed that it was causing CSA's. I had nasal airflow issues that required a 3rd nasal repair (for collapsed internal valves not noticed by the first 2 Kaiser surgeons !), & other treatments (septal plug, nose oil to prevent crusting causing nasal blockages). Even those remedies took a LOT of persistence as I worked my way thru an ENT/HNS dept that wrongly wanted to do a 3rd septoplasty, and was "resistant" to my persistence. There remained some issues about which they were clueless. I have a very low opinion of some policies, procedures, & personnel within Kaisers ENT/HNS dept, but there are good people as well. After I had painstakingly eliminated all other possibilities (reflux, cardio/pulmonary issues, standard OSA/CPAP treatment, resolved most of the nasal issues), and eventually paid over 3k out of pocket for the sleep study at a hospital (which proved that APAP, BI-PAP & ASV ALL caused massive central apneas and that oxygen was the only thing that helped), eventually Dr Richey was willing to experiment with oxygen at night (via an oxygen concentrator). Success !! Why do I tell you all this ? Because it illustrates that each dept at Kaiser, and even practitioners WITHIN a dept such as ENT/HNS, is in their own "silo". Seemingly unwilling to communicate with other depts, or even sub-specialists within their dept, unable to consider that some aspects of a medical condition might require treatment by other depts, and seemingly unable to consider anything other than their most standard dx & treatment - because there are huge gaps in their knowledge base. I rate Dr Richey with 3.5 "stars" because she was always very polite, even when I was being rather "pushy". I admit that I am not always skilled at being assertive in a polite way, and if I were in her position, I would have been irritated & frustrated by me. At each step of my relentless multi dept persistence on various treatments & tests, and based on what I had managed to prove at each point along the way, Dr Richey DID eventually approve the next logical, though not always imaginative, step of testing and treatment. She deserves credit for that, and I appreciate her help. During this journey my co-pays & out of pocket expenses cost about 10k. And that was with a "pretty good" insurance plan. And 4 extra years of NO restorative sleep & very low nocturnal oxygen levels. Medical lesson # 1 : Sadly my friends, that level of treatment is all we can expect of our "best in the world" medical system. (I rate Kaiser as at best 2 stars out of 5). Medical lesson # 2 : In order to "thrive" at Kaiser (and probably any health insurer or medical provider), you need to be a very assertive and persistent advocate for yourself."
